This questionnaire is designed to give us a thorough assessment of your pet's health. We want to assure you that we DO NOT share, sell, or disclose ANY information we gather. Important details like your pet's breed, age, sex, approximate weight, and activity level are essential for determining the right nutrients to ensure a healthy and happy life. If your pet has any known health issues, please let us know the diagnosis along with any relevant observations you've made. Of course, we will need your contact information to discuss our findings. The cost for this service is $25.00 US, and in return, we will analyze the information and provide a written evaluation of your pet's overall nutritional requirements, the amount to feed at each meal, necessary nutrients or supplements if needed, and a complimentary phone consultation for any further questions. If you're interested in 'Home Cooking' for your pet, we can also offer recipes for an extra fee.
Please take a moment to fill out the form as accurately as you can. The more detailed the information you provide, the more thorough our assessment will be, enabling us to offer you better recommendations. After completing the form, kindly email it to info@getrxnutrition.com. Alternatively, you can text your responses to 727-389-6119 or leave a message with your details on my voicemail. For a complete assessment, I require at least the breed, age, sex, and weight, as these details are crucial for calculating feeding amounts in our suggestions. Typically, we aim to provide you with a recommendation within 2 to 3 business days. If your situation is urgent due to illness or injury, please give us a call, and we will do our best to prioritize your request.

In 2014, the FDA released a report discussing Dilated Cardiomyopathy (DCM) in both dogs and cats. This report was compiled from complaints made by pet owners and veterinarians who suggested that DCM diagnoses were associated with grain-free diets. DCM is a serious condition that typically manifests around the age of 6, characterized by an enlarged heart and symptoms that can include coughing, lethargy, loss of appetite, and even sudden death. I have attached the original report along with the specific complaints on our FAQ page (DCM Report). In the following years, I conducted my own research on this condition. The report does not explicitly identify certain food brands as being connected to DCM.
“No one food was named as an issue, nor was there any mention of recalls by any manufacturer”
The complaints detailed the pets' breed, age, sex, weight, and diet, while the veterinary records also contained symptoms and, in some instances, the actual lab report. I recorded all that information for the 1,135 pets on the list (with only 8 being feline). After compiling the data, I organized it into various categories. It became evident that there was a genetic factor involved in the condition. Although the range of diets and manufacturers was too diverse to pinpoint a common cause, the majority of patients fell into the same category, with one breed or its derivative accounting for more than half of all cases.
Summary
The findings showed that Golden Retrievers topped the list, followed closely by Labrador Retrievers, and then mixed breeds that included some retriever, accounting for over half of all the dogs listed. The other breeds were mainly large breeds, but they were more evenly spread across different types. Small breeds had minimal representation, with most being under 1%. In my opinion large breeds, especially retrievers, should be fed a rice formula. The numbers from my research are on the FAQ page (FDA Report sorted by Breed)
